We are a Non-profit youth football organization. Our reason for developing this organization
was to bring the best youth football program possible to our children and to our community.

I hereby release the Hallsville Football & Cheer Association (HYFCA), the Board of Directors, Coaches, Assistants, and Volunteers from all claims or accidents not included in the Hallsville Football & Cheer Association Insurance Policy. This release is for my child for all activities directed by the Hallsville Football & Cheer Association. Furthermore, I agree to authorize the Hallsville Football & Cheer Association, the Board of Directors, Coaches and/or Assistant Coaches to seek proper medical treatment for my child in the event I cannot be reached or notified at the time of the emergency or required medical treatment.

I will enjoy my child’s opportunity to experience the benefit of the sports; trust in my child’s ability to have fun as well as to perform and achieve excellence on his/her own; help my child to learn the right lessons from winning and losing and from individual accomplishments and mistakes; respect my child’s teammates and fellow parents as well as the players, parents and coaches from opposing teams; give only encouragement and applaud only positive accomplishments whether for my child, his/her teammates, their opponents of the officials; respect my child’s coach and support his/her efforts; not instruct from the sidelines unless asked by the coach; respect the officials and their authority during games; Never demonstrate threatening behavior or use foul language. You will be asked to leave immediately.

By signing below, I understand that all rules, regulations, and guidelines of the Hallsville Football & Cheer Association and the East Texas Independent Youth Cheer League will be followed and supported by all Parents/Guardians and students. I agree that participating in Hallsville Football & Cheer Association will require me to agree to help my child participate in the fundraiser(s) in order to provide for a successful season. I accept responsibility for the return of any equipment belonging to Hallsville Football & Cheer Association at the close of the season or I will be held financially responsible for the replacement cost of said equipment.

I will: treat all coaches, officials, parent volunteers, teammates, and opposing players in a respectful manner; display good sportsmanship at all times on and off the field; attend scheduled practices and games and notify the coach when attendance is not possible; participate fully in all practice and game activities unless such activity is restricted through parental consultation with the coach; inform coaches of any injury sustained prior to or during practices and games.

("the volunteer") releases Hallsville Youth Football & Cheer Association (“HYFCA”), and each of its board of directors, officers, and agents. The Volunteer desires to provide volunteer services for HYFCA and engage in activities related to serving as a volunteer mentor/coach/support.

All volunteers understand that the scope of volunteering with HYFCA is limited to a volunteer position and that no compensation is expected in return for any and all services provided by volunteer; that HYFCA will not provide benefits traditionally associated with employment to volunteers; and that volunteers are responsible for his/her own insurance coverage in the event of personal injury or illness as a result of volunteering service to HYFCA.

I, the volunteer, release and forever discharge and hold harmless HYFCA and it successors and assigns from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from the services I provide HYFCA. I understand and acknowledge that this Release discharges HYFCA from any liability and/or claim that I may have against HYFCA with respect to bodily injury, personal injury, illness, death, or property damage that may result from the services that I elect to provide to HYFCA or occurring while I am providing volunteer services.

Further the Volunteer understands that HYFCA does not assume any responsibility for or obligation to provide the volunteer with financial or other assistance, including but not limited to medical, health, or disability benefits or insurance of any nature in the event of my injury, illness, death, or damage to my property. I expressly waive any such claim for compensation or liability on the part of HYFCA beyond what may be offered freely by HYFCA in the event of such injury or medical expenses incurred to me.

The Volunteer releases and forever discharges HYFCA from any claim whatsoever which arises or may hereafter arise on account of any first-aid treatment or other medical services rendered in connection with an emergency during the tenure as a volunteer with HYFCA.

The Volunteer understands that the services provided by HYFCA may include activities that may be hazardous including but not limited to extreme weather conditions, expose to chemicals, and uneven surfaces involving inherently dangerous activities. As a volunteer, I hereby expressly assume the risk of injury or harm from these activities and any other hazardous activity, and release HYFCA from all liability for injury, illness, death, or property damage resulting from the services I elect to provide as a volunteer or occurring while I am providing volunteer services.

The Volunteer grants and conveys to HYFCA all rights, titles, and interests in any and all photographs, images, videos, or audio recordings of me or my likeness or voice made by HYFCA in connection with my providing volunteer services to HYFCA.

As a volunteer, I expressly agree that this release is intended to be as broad and inclusive as permitted by the laws of the State of Texas and that this release shall be governed by and interpreted in accordance with the laws of the State of Texas. I agree that in the event any clause or provision of this Release is deemed invalid, the enforceability of the remaining provisions of this Release shall not be affected.
